Please fill out the waiver registration for EVERY member in your family using the form below if you are interested in participating (skating or observing) in our WOC Ice Rink. Only family members may fill this out on behalf of other family members. This waiver is good from November 1, 2024 to January 31, 2025, so only one form is needed to be filled out per family member once.

IMPORTANT NOTES: Children must be three years old to skate. Adults bringing children 12 and under must fill out a waiver to assist children with equipment.
